Make sure you have already created a Service. To create a Service, read this.

Also, make sure you have saved your service request form as a template. Not sure how to do it? See here.

On your Dashboard, go to Services > All Services

Click on Create Service

Add in the service type, service details, and pricing details

Click on Next

On the Request form, click on the vertical three dots

Click on Import from Template

Select the desired template, click import

Once the form has been populated, feel free to tweak to your liking

Once done, click on Save
