How to customize service request forms

Build the request form clients fill in for each service: field types, field options, preview, conditions and templates.

Written by Gabriel Lecointere

Use case: a logo request needs different details than a blog post. Each service has its own request form, so you collect the right brief from your clients before work starts.

The request form is what clients fill in when they submit a request for a service. You need to be a portal admin or admin to edit it.

Open a service's request form

  1. Go to Services > All services.

  2. Click the three dots next to the service and choose Edit request form. To build a form for a new service, click Create service instead and follow the steps until you reach Request form.

The Request form step has two tabs: Form body, where you build the form, and Conditions, where you show or hide fields based on answers.

Build the form

Every form starts with a title field. Clients use it to name their request, and the name is used everywhere in your portal. Click the label to rename it, for example to Project title.

  1. Click Add item at the bottom of the form to add a field.

  2. Click a field to edit it. Click its label to rename it.

  3. Choose the field type in the dropdown next to the label.

  4. For selection fields, click Add option to add choices.

  5. Turn on Required if clients must answer it.

Changes to fields are saved as you make them. Click Save at the bottom when you're done to save the service.

Several choices from a list.

Field options

When a field is selected, its toolbar shows the options that apply to its type:

  • Switch to image selection (picture icon): on Single selection and Multiple selection fields, show each option as an image card. Upload an image per option.

  • Duplicate: copy the field.

  • Repeatable (arrows icon): on Short answer, Paragraph, Date and Rich text + image fields, let clients add several answers, for example several sizes or addresses.

  • Billable (dollar icon): shows once a field is repeatable. Each extra item the client adds is charged at the service price. This only works with one-off services.

  • Remove: delete the field.

  • The three dots: Show help text adds a line of guidance under the label.

For more on repeatable, billable and dropdown fields, see Advanced request form fields: internal, repeatable, billable, dropdown.

Preview the form

Click Preview at the top of the form. It opens in a new tab as your clients will see it, with conditions applied. It updates as you edit.

Show or hide fields with conditions

Open the Conditions tab to show or hide a field based on an answer, for example show Banner sizes only when the client picks Banner. See How to use conditional rules for your request forms. Plan your fields first; conditions are easier to set up once all fields exist.

Save a form as a template

Templates save you from rebuilding the same fields for each service. They're shared across all services in your portal.

  1. On the Request form step, click the three dots next to Preview.

  2. Click Save as template.

FAQ
